Located at the reception lobby of St. Luke’s Hospital (SLH), CareConnect@SLH began in January 2007 as a one-stop resource centre on caregiving to the elderly. This service complements SLH’s inpatient and outpatient services.

Who is it for?
Caregivers of the elderly
Seniors who require community support, while residing at home
Seniors poised for graceful and successful aging
Professionals working with seniors
Volunteers who assist seniors within the community

Visitors may obtain information and general advice relating to:
Issues of caregiving
Preventive health strategies
Illness and disease management for the elderly (e.g. stroke, diabetes, high blood pressure, mental health)
Community support services (e.g. daycare centres, day rehabilitation centres and other support services)
Equipment and activities of daily living aids

Caregivers with questions on the above have addressed their concerns at CareConnect@SLH and were better prepared for their roles in caregiving.
