Job Summary:

  • Facilitate and conduct Advance Care Planning discussion with patients and next of kin at the inpatient, outpatient and community settings
  • Conduct talks or workshops to raise awareness and advocate for Advance Care Planning

Responsibilities and Duties:


  • Process and manage referrals for ACP discussions from inpatient, outpatient and community settings of SLH
  • Facilitate ACP discussions with patients, families and nominated spokespersons
  • Document ACP discussions at the appropriate platforms
  • Coordinate between patients, families and healthcare team for ACP facilitation and follow up
  • Make referrals to community services related to ACP matters


  • Work with ACP Coordinator to provide support in ACP referrals and enrolment and data input as needed
  • Conduct talks and workshops for healthcare professionals to raise awareness about and advocate for ACP
  • Participate in public awareness events (within St Luke’s Hospital or jointly with partners) to raise awareness about ACP
  • Participate and assist in conducting audits, research, surveys or quality improvement projects related to ACP
  • Preparation of reports and statistics as required.

Job Requirements:

  • Professional qualification in Nursing, Social Work, Allied Health or/and any clinical disciplines relevant to the job.
  • At least 3 years of experience as an Allied Health Professional
  • Experience in a Hospital setting preferred
  • Certification in ACP Facilitation (General and Preferred Plan of care) and experience in conducting ACP discussions would be an asset
