Support Programme
Established in March 2019, the Nursing Home Support Programme aims to collaborate with nursing home partners to provide additional clinical support and improve the quality of life for residents.
Nursing Home Support Programme Initiatives
Medical consultations (on-site and tele-medicine)
SLH Doctors provide timely and appropriate medical interventions to residents of nursing homes through regular on-site medical consultation and up to 14 hours of daily tele-consultation.
There will be regular multi-disciplinary reviews which help to strengthen communication and reinforce good clinical practices.
Speech Therapist Services
SLH Speech Therapists provide inputs and timely intervention for residents with swallowing difficulties to improve their overall quality of life. They also advise on texture modified diets to strengthen the nursing home’s food services.
Training for Nursing Home Staff
SLH Doctors and Speech Therapists conduct regular educational talks on care plans and interventions, and clinical topics that are relevant to nursing home staff. These talks are also open to other nursing homes within the same network of the nursing home group.
