Chronic wounds affect at least 150,000 patients in Singapore annually. The four most common chronic wounds are pressure injury, diabetic foot ulcers, chronic venous ulcers and arterial leg ulcers. With an ageing population, the need to manage chronic wounds in the community and Intermediate and Long-Term Care (ILTC) facilities is ever more pressing.
How it works
Our dedicated wound clinicians (nurses) who are specialised in wound care management will provide consultancy to your healthcare team. They will share their analysis, expertise and guide nurses, on-site at your facility or virtually.
The consultancy done in 3 stages
1. Pre-assessment
- Provide initial assessment on staff wound care knowledge
- Assessment on type of wound cases
- Assessment on inpatient skin/wound care management
- Assessment on wound care product inventory
- Assessment on wound data repository
2. Consultancy
- On-site/virtual consultancy of wound cases
- On the job training for care staff
- Develop assessment framework to assess staff comptency
- Develop management and escalation protocols and guidelines
- Conduct weekly wound rounds, mentor and supervise nurses so as to enhance their knowledge and skill sets for management of complex wounds
- Case discussion on complex cases
3. Post assessment
- Report on the cases seen, analysis, work done, outcomes in report
- Recommend additional training for staff if required

The consultancy done in 3 stages
1. Pre-assessment
- Assessment of staff competency and knowledge
- a. Speech Therapists will provide consultancy on food service by assessing the menu to and assess the modified diet consistencies for elders with dysphagia adhering to International Dysphagia Diet Standardisation Initiative (IDDSI).
2. Consultancy
- Speech Therapy
- Provide training to staff on safe feeding and managing elderly with dysphagia (swallowing impairment) and communication disorder and other conditions
- Set up regular audit for modified diet consistency
- Advisory role to kitchen staff/dietician on modified diet consistencies
- Physiotherapy / Occupational Therapy
Provide support for community facilities, and ILTC facilities (including nursing homes) by deploying our team of physiotherapists and/or occupational therapists on-site.
3. Post assessment
- Report on the cases seen, analysis, work done, outcomes in report
- Recommend additional training for staff if required
