The giving of such data is considered consent to the collection, use, disclosure and storage of such data for the purposes related to the provision of medical care to you. These purposes include referrals to other healthcare professionals and institutions, and other purposes including but not limited to billing and hospital business such as informing you about our services and activities. Such business may continue after discharge, including but not limited to outpatient care, home care and financial matters.
