Our dementia care service is committed to supporting persons living with dementia and their caregivers through a comprehensive, person-centred approach. We aim to nurture an environment where individuals and families can face the challenges of dementia with dignity and confidence.

Our multidisciplinary team works closely with families to provide specialised care, addressing behaviours of concern and offering practical strategies to cope both at home and within the community. Understanding that every patient is unique, we design tailored programmes to enhance cognitive function, promote emotional well-being, and improve overall quality of life.

Our key services include consultations with family physicians, occupational therapy with a focus on cognitive rehabilitation and behavioural management, and caregiver education and training to equip families with essential knowledge and skills. Additionally, we offer care coordination and referrals to community resources, ensuring seamless and continuous support that allows persons with dementia to age well in place.

As a CHAS (Community Health Assistance Scheme) accredited clinic, your out-of-pocket payments for chronic disease follow-up consultations, medications and treatments are subsidised.

Essential Health Support

The MediSave scheme in Singapore helps individuals manage outpatient care expenses under programs like MediSave500 and MediSave700, which offer financial support for treatments such as managing chronic conditions. These programs have withdrawal limits to ensure funds are used sustainably while covering essential medical needs.
