Diabetes Care Service

Diabetes Care Service
Diabetes has become a critical and escalating health issue in Singapore, affecting quality of life and potentially leading to severe complications if not properly managed. In response, the Ministry of Health has initiated a nationwide campaign aimed at preventing diabetes, promoting early detection, and ensuring effective management to help Singaporeans maintain healthier lifestyles.
Our multidisciplinary team is dedicated to offering comprehensive, patient-focused support for individuals living with diabetes. Our holistic care strategy encompasses a variety of specialised services, including:
- Consultations with family physicians
- Nutritional guidance through dietician counselling
- Disease education and self-management support from diabetic nurse counsellors
- Professional continuous glucose monitoring to empower diabetics to make lifestyle changes
- Screening for diabetic kidney disease
- Screening for diabetic foot complications
- Early detection of eye-related issues via diabetic retinal photography
- Management of wound care
Through a comprehensive approach, we strive to engage and empower individuals with diabetes to take control of their condition and enhance their overall health and well-being.
