Lasting Power of Attorney (LPA) Certification
What is LPA?
The LPA is a legal document which allows a Donor who is at least 21 years of age, to voluntarily appoint one or more Donee(s) to make decisions and act on his/her behalf if he/she loses mental capacity one day, in broad areas of personal welfare and property & affairs matters.
Our medical practitioners, accredited by the Public Guardian, act as Certificate Issuers to certify and witness the Donor’s signing of the LPA Form.
Before visiting our medical practitioners, please ensure the following have been completed via the Office of the Public Guardian website:
- Donor completes the LPA Form
- Donee(s) accepts appointment
