Intervention Team
St Luke Hospital’s Community Intervention Team (SLH COMIT) is an allied health-led, multi-disciplinary team that aims to provide holistic services for clients with mental health issues, including dementia, and their caregivers at home.
Programme Objectives and Benefits
- Carry out community management of dementia, suspected dementia and diagnosed mental health issues (for instance, depression or anxiety) through assessment, treatment and interventions, care coordination and service linkages
- Enable Persons Living with Dementia and Persons with Mental Health Issues to remain in the community for as long as possible, while maintaining or enhancing their quality of life
- Support caregivers in their caregiving journey through education, psychosocial support and linkages to community services
We can help:
Persons living with dementia, suspected dementia or persons living with mental health issues, aged 40 and above, and residing in the western region.
Who
- Persons diagnosed with dementia exhibiting challenging behaviours
- Suspected dementia (requiring assessment and management)
- Adults (40 years old and above) diagnosed with mild to moderate mental health conditions (e.g. depression, anxiety)
- Caregivers experiencing high level of caregiving stress or facing challenges in care
- Residing within the following areas:
