Intervention Team

St Luke Hospital’s Community Intervention Team (SLH COMIT) is an allied health-led, multi-disciplinary team that aims to provide holistic services for clients with mental health issues, including dementia, and their caregivers at home.

Programme Objectives and Benefits

  • Carry out community management of dementia, suspected dementia and diagnosed mental health issues (for instance, depression or anxiety) through assessment, treatment and interventions, care coordination and service linkages
  • Enable Persons Living with Dementia and Persons with Mental Health Issues to remain in the community for as long as possible, while maintaining or enhancing their quality of life
  • Support caregivers in their caregiving journey through education, psychosocial support and linkages to community services

We can help:

Persons living with dementia, suspected dementia or persons living with mental health issues, aged 40 and above, and residing in the western region.

Who

  • Persons diagnosed with dementia exhibiting challenging behaviours
  • Suspected dementia (requiring assessment and management)
  • Adults (40 years old and above) diagnosed with mild to moderate mental health conditions (e.g. depression, anxiety)
  • Caregivers experiencing high level of caregiving stress or facing challenges in care
  • Residing within the following areas:
    • Bukit Batok East
    • Bukit Batok SMC
    • Bukit Panjang SMC
    • Cashew
    • Clementi
    • Hong Kah North SMC
    • Jurong Central
    • Jurong Spring
    • Ulu Pandan
    • West Coast
    • Yuhua SMC
