Intervention Team
St Luke’s Hospital Community Intervention Team (SLH COMIT) is a community-based programme where allied health professionals (nurses, occupational therapists, medical social workers) support Persons Living with Dementia and their caregivers at home.
Programme Objectives and Benefits
- Carry out community management of dementia, suspected dementia and mental health conditions (depression, anxiety) through assessment and diagnosis, treatment and intervention, care coordination and service linkages
- Enable Persons Living with Dementia to remain in the community for as long as possible
- Support caregivers in their caregiving journey through education, psychosocial support and linkages to community services
We can help:
Persons with dementia, suspected dementia or mental health conditions (depression, anxiety), aged 40 and above, and residing in the western region.
Who
- Persons diagnosed with dementia exhibiting challenging behaviours
- Suspected dementia (requiring assessment and management)
- Adults (40 years old above) with mood disorders (eg depression, anxiety)
- Caregivers experiencing high level of caregiving stress or facing challenges in care
- Residing within the following catchment areas:

- Bukit Batok SMC
- Hong Kah SMC
- Jurong Central GRC
- Zheng Hua GRC
- Yu Hua SMC
- Jurong Spring GRC
- West Coast GRC
- Cashew & Ulu Pandan GRC
