Inpatient Care
Our multi-disciplinary care team provides rehabilitative and sub-acute care through our general wards and specialised wound, dementia and palliative wards.
Outpatient Care
Our outpatient services, including St Luke’s Community Clinic and Day Rehabilitation Centre, provide continuity of care to meet community healthcare needs.
Home Care
Our home-based services, including home medical, home nursing and home therapy, provides holistic care for home-bound clients.

For I-CARE PCN only:
Referral criteria:
Any of the following symptoms:
- Patient with frequent falls or any recent falls
- Deconditioned, CFS >3
- Any musculoskeletal problems – eg frozen shoulder, knee pain, back pain
For all referrals:
- Please fill up PCN Patient Services Referral Form
- In the form, indicate “SLH DRC” under Reason for Referral

Referral criteria:
Homebound seniors with chronic conditions or disabilities who require continuing or long term medical care.

Referral criteria:
Any of the follow
- CFS 2 to 5
- Medically stable
- Not experiencing any form of physical pain that limits their movement
