What You Can Expect

1. Assessment

A doctor will assess your suitability and provide counselling upon enrolment about the service.

2. Baseline Screening & Support

Through a holistic baseline screening, we evaluate your physical, psycho-emotional and spiritual health and guide you on our after-clinic support to help you continue your care confidently.

3. Reviews

You will have regular follow-up reviews with our medical, nursing and chaplaincy team every 1–3 months, depending on your needs.

PPCC is Singapore’s first palliative care clinic embedded in primary care. Unlike most palliative services, which are based in hospitals, inpatient hospices, or home care, PPCC offers care directly within the community clinic setting for easier access and continuity.

PPCC focuses on early palliative care, not just at the later stages of illness. Starting early allows for proactive care, enhancing comfort and quality of life. As your needs evolve, you can seamlessly transition to inpatient hospice or home palliative care within St Luke’s Hospital’s palliative ecosystem.
