Diabetes Care Service

Diabetes Care Service

Diabetes has become a critical and escalating health issue in Singapore, affecting quality of life and potentially leading to severe complications if not properly managed. In response, the Ministry of Health has initiated a nationwide campaign aimed at preventing diabetes, promoting early detection, and ensuring effective management to help Singaporeans maintain healthier lifestyles.

Our multidisciplinary team is dedicated to offering comprehensive, patient-focused support for individuals living with diabetes. Our holistic care strategy encompasses a variety of specialised services, including:

  • Consultations with family physicians
  • Nutritional guidance through dietician counselling
  • Disease education and self-management support from diabetic nurse counsellors
  • Professional continuous glucose monitoring to empower diabetics to make lifestyle changes
  • Screening for diabetic kidney disease
  • Screening for diabetic foot complications
  • Early detection of eye-related issues via diabetic retinal photography
  • Management of wound care

Through a comprehensive approach, we strive to engage and empower individuals with diabetes to take control of their condition and enhance their overall health and well-being.

Continuous Glucose Monitoring (CGM)

Continuous Glucose Monitoring (CGM) is a smart and convenient way to track your blood sugar levels in real time — day and night. Whether you’re managing diabetes or simply optimizing your health, CGM offers powerful insights that go beyond traditional fingerstick testing. A small sensor is placed just under the skin to measure glucose levels in the fluid between your cells. It then sends the results to a device — like your phone, an insulin pump, or a small monitor — so you can see your levels anytime, anywhere.

Benefits of CGM Programme

  • 24/7 real-time blood sugar monitoring
  • Instant alerts of dangerous highs (hyperglycemia) or lows (hypoglycemia)
  • Early detection of patterns and trends
  • Better decision-making around food, exercise, and medication
  • Reduces the need for finger-stick testing
  • Tailored health support plans

Who is CGM for?

Our CGM programme is suitable for:

  • Individuals with type 1/type 2 diabetes
  • Individuals with poor or difficult control of diabetes
  • Individuals with pre-diabetes or newly diagnosed diabetes
  • Anyone looking to optimise their health, diet, or fitness through better glucose awareness

Ready to Take Control of Your Blood Sugar?

Join our CGM programme and get the tools, insights, and support you need to make smarter health decisions — every day.
